Job Description

CCHOHT Overview:

The Connected Care Halton Ontario Health Team (CCHOHT) is one of the first OHT teams in the province that is working to implement a new model of organizing and delivering health care that better connects patients and providers in their communities to improve patient outcomes. The Ontario Health Team (OHT) model is part of the Ontario Ministry of Health's (MOH) health system transformation strategy. The CCHOHT is guided by a Collaborative Committee with representatives from Halton Healthcare, Halton Region, Ontario Health at Home, Physicians, PFAC and Community Organizations.

*Please note: This position is funded by Ontario Health and will report into the leadership of the Connected Care Halton Ontario Health Team. Halton Healthcare is supporting this recruitment as the operational support partner of the CCHOHT.

This is what youll be doing:

The CCHOHT is seeking a Project Manager to support the advancement of the Primary Care Network (PCN) and the successful achievement of Key Performance Indicators (KPIs) as set out through the Funder (Ontario Health) for the phased rounds of PCAT funding through year 1. The ideal candidate will have experience in the primary care sector, knowledge of integrated health care systems, and the ability to work collaboratively with the leads of successful proposals and their partners through Interprofessional Care Team Expansion funding initiatives provided by Ontario Health.

The incumbent will work as a member of the CCHOHT to provide the following:

  • Supports the development of project milestones and KPIs focused on access, attachment and enablement of new patients to existing and net new primary care teams over year one within the CCHOHT defined forward Sortation Areas (FSAs).
  • Ensuring Engagement Capable environments framework and involvement of Patients, families and caregivers are embedded throughout planning, implementation and evaluation of new and expanded teams.
  • Project Management for implementing new models of Interprofessional care teams as they are established through a primary care action team.
  • Ensuring embedding health equity principles in all co-design.
  • Oversees reporting requirements through the TPAs through to the CCHOHT as per requirements from Ontario Health.
  • The oversight and deliverables of PCAT proposal submissions from primary care partners for new/expanded interprofessional primary care team (IPCT) investment inclusive of FHT/FHO new and expanded team approved proposals as they are granted.
  • Achieving success on Provincial directives to develop and implement a plan to facilitate attachment of patients currently on the Health Care Connect waitlist by spring of 2026.
  • Functions as the point of contact for the Health Care connect waitlist attachment strategy for the CCHOHT
  • Project milestones and progress reporting for year one through the eventual goal of 100% attachment of local population by 2029, based on identified primary care attachment and access gaps and aligned to provincial direction.
  • collaborative initiatives between the CCHOHT and partner members for clinical services for unattached patients while they await attachment to primary care.
  • The advancement of digital priorities supporting the Clinical Lead, Digital Projects.
  • The advancement Social Prescribing initiatives.
Qualifications
  • Project Management (PMP) Certification considered a Strong Asset
  • Degree in a regulated healthcare field considered an asset
  • Masters degree in Business, Health Administration program or related field preferred
  • Direct Experience in the healthcare/ Primary Care Sector
  • 3-5 years of practical and related experience
  • Experience launching successful change initiatives within a healthcare setting
  • Key partner engagement, project planning, management and implementation experience required
  • Change Management & Relationship Management experience required
  • Experience with successful Digital Health/IT initiatives an asset
  • Demonstrated knowledge and application of quality improvement principles (e.g. Lean Methodology) as asset
  • Excellent written, oral communication skills and Interpersonal Skills
  • Excellent Time Management, meeting timelines
  • Ability to work as a team, professional and courteous manner
  • Exercise good judgement, initiative, with ability to set appropriate priorities
  • Knowledge of Ontario Health Teams, Primary Care Networks, Primary Care Action Plan and healthcare transformation
  • Knowledge of primary care models
  • Training and commitment in EDI
  • Quintuple Aim Framework, QI and PM frameworks and tools
  • MS Teams, Microsoft Word, Outlook, Excel, Power Point, Visio
Additional Information

Status : Temporary Full-Time, 1 year (12 months) contract

Hours : Monday to Friday - 7.5 hour day shifts (Hours are subject to change based on operational needs).

Salary: $53.083 to $65.767 per hour
